Is there a fee to use MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health.With MyChart, you can use the Internet to:

  • Appointment requests for all specialties
  • Real-time appointment scheduling with a primary care physician
  • Prescription refill requests
  • Mammogram Results
  • Access your personal and family's medical records
  • Message your physician's office
  • View lab results
  • View your CHI Health Bill

What do I need to use MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account must be accessed using that password. On mobile, the account may also be accessed using face ID or fingerprint login. Unlike conventional email, all MyChart messaging is done while you are securely logged in to our website or mobile app.

You can ensure that your account stays secure even if someone else has your username or password by turning on two-step verification. When this feature is turned on, you must enter a code that is sent to you by email or text message to log in to MyChart, in addition to using your username and password. You might receive email or text notifications that contain links to MyChart. These notifications allow you to login to MyChart securely using a link that expires after a short time. Keeping your MyChart account secure means keeping your phone number and email account secure. If you think that your phone number or email account may be compromised, you can change the phone number or email account on file on the "Personal Information" page in MyChart.

Enrollment Questions

How do I sign up?

Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may call your primary care clinic to get one, ask to sign up during your next office visit or sign up online.

Are there other ways to access MyChart?

Yes. You can use another application to access your health information, if you prefer.

  • Currently available is the Health app, which is offered by Apple Inc. for iPhones with the current software version. You will recognize it easily by the heart image. The Health app consolidates all of your health data from iPhone, Apple Watch, and third-party apps you already use, so you can view all your progress in one convenient place. This includes your health records from CHI Health and other health systems if you choose to add them. To get started:/li>
    • You will need to first set up a portal account with MyChart.
    • The username and password you choose will be the security component for your health information and make it possible for you to access and import it.
    • Please note: Catholic Health Initiatives reserves the right to disable a third-party application, if needed. That application’s terms of use will apply and CHI will not have the control or ability to secure the data you place within that system.
  • Patients of providers that use Epic software are also able to connect other 3rd party applications (apps) to retrieve parts of their health record for their own personal use. Examples of data that can be pulled into an app include lab results, allergies, medications, and immunization history. In order to authenticate a 3rd party app to retrieve your health data, follow these steps.

Can you send me a new activation code as I have lost it, let it expire or did not receive it?

To request a new activation code on MyChart, you may call your primary care clinic to get one, ask to sign up during your next office visit or sign up online.

My activation code does not work, what should I do?

For your security, your activation code expires after 14 days and is no longer valid after the first time you use it. If you still have problems, you may call our MyChart support line at 844-528-8383 with any questions.

Your Medical Record

When can I see my test results in MyChart?

Your test results are released to your MyChart account after your physician has reviewed them. This is generally within 3 business days.

Why are certain test results not shared electronically via MyChart?

Your provider is able to determine which types of test results are able to be accessed through MyChart. Further, tests of a very sensitive nature are not released to MyChart.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 1-3 business days. Please note that MyChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

Where can I update my personal information (e.g., home address, e-mail, or change my password)?

Once logged into MyChart, select My Account followed by Notifications. You can update your e-mail address under the Contact Information section.

MyChart for My Family

Can I view a family member's health record in MyChart?

Yes you can. This is called Proxy access and allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form and return it to one of our medical facilities to request access to this convenient service.

Can I ask questions regarding a family member from my MyChart account?

MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must establish their own MyChart account.

You may click the "Forgot password" link on the sign-in page to reset your password online.

I was logged out of MyChart, what happened?

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

What is your Privacy Policy?

MyChart is owned and operated by MyChart and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.
